At FOCUS Connect, we are a not-for-profit charity and an approved My Aged Care provider committed to enhancing the lives of seniors in Sydney’s Northern Suburbs. Through the Home Care Package program, we offer Social Support services to eligible recipients, helping you remain active and engaged in your community.
HCP Social Support for Individuals
Social connection is essential for overall wellbeing. At FOCUS Connect, we provide social support to help you stay engaged and involved in your community. This service is available to eligible Home Care Package recipients in Sydney’s Northern Suburbs, including Ryde, Hornsby, Epping, Pymble, Eastwood, and surrounding areas.
We offer companionship through regular visits, outings to local events, and connections to interest groups. These services are designed to help you maintain important social ties and enjoy meaningful interactions.
Transportation services are also available, giving you greater independence without relying on friends or family for travel. A dedicated support worker will be paired with you to provide companionship and help you stay connected with your community.
