The process to swap My Aged Care providers is designed to be manageable, ensuring that participants can transition smoothly without compromising their care or well-being. Below is a step-by-step guide on how to approach swapping My Aged Care providers:
Research and select a new aged care provider that meets your needs. Check your current Home Care Agreement for any notice periods or exit fees that may apply.
Write a letter to your current provider informing them of your decision to switch. Specify the notice period as per your existing Home Care Agreement.
Request written confirmation from your current provider regarding the last date they will provide care, known as your cessation date.
Call My Aged Care at 1800 200 422 to inform them of your intention to transfer your Home Care Package to a new provider. Obtain your new Unique Referral Code during this call.
Share your Unique Referral Code with your new provider. This code allows them to access your My Aged Care records and register as your new care provider.
Work with your new provider to develop a new Care Plan and Agreement. Any unspent funds from your current package will be transferred to your new provider to support your ongoing care needs.
