Summary:

We are seeking an experienced Aged Care Coordinator to manage and support a caseload of Aged Care consumers under the Commonwealth Home Support Programme. The successful candidate will be responsible for planning and implementing a client-focused model of care, responding to enquiries, conducting assessments, developing care plans, supporting clients in need of a care review, supervising support workers, maintaining positive stakeholder relationships, ensuring compliance with funding and service agreements, managing incidents, feedback and complaints.

About the Role:

As a Care Coordinator, you will be responsible for the following but not limited to:

  • Manage a caseload of Aged Care Commonwealth Home Support Programme consumers.
  • Support the planning and implementation of a client-focused model of care aligned with Aged Care Quality Standards.
  • Conduct face-to-face annual, and change-of-circumstance assessments
  • Develop appropriate care plans that align with client goals, diverse needs, wellness principles within approved services.
  • Collaborate with the rostering team to ensure efficient scheduling of Support Workers in line with organizational policies.
  • Provide ongoing supervision for Support Workers and conduct annual performance reviews.
  • Provide feedback on systems improvement to supervisor for compliance within funding/service agreements.
  • Aim to meet service delivery targets as agreed upon in the CHSP activity plan.
  • Build positive relationships with government agencies and service providers involved in aged care services.
  • Work closely with the Aged Care administration team regarding client statements, budgets, invoices processing.
  • Ensure client documentation is current and accurate
  • Manage compliments/complaints/feedback from clients; identify areas for improvement.
  • Ensure effective incident report response and report any reportable incidents to Supervisor or Aged Care Manager.
  • Monitor quality and support compliance within the ISO27001 Framework.
  • Promote effective relationships with external service providers, ensuring compliance with Aged Care Standards, best practices, funding arrangements, and service agreements.
  • Complete additional tasks assigned by the Senior Care Coordinator or Manager.

Your Skills:

  • Strong understanding of the Aged Care Quality Standards
  • Excellent communication and interpersonal skills
  • Ability to effectively manage a caseload of clients
  • Excellent organisational skills
  • Proficient in conducting assessments and developing care plans
  • Knowledgeable about the Commonwealth Home Support Programme
  • Desirable skills of experience in rostering support workers
  • Familiarity with ISO27001 Framework for monitoring quality compliance
  • Good problem-solving abilities
  • Ability to build positive relationships with key stakeholders

Essential Criteria:

  • Current and valid Working With Children Check clearance (or willing to obtain)
  • Current and valid full Driver’s Licence and access to a comprehensively insured vehicle
  • Current and valid First Aid Certificate (or willing to obtain)
  • Cleared National Criminal History check (or willing to obtain)
  • Experience working within Aged Care (or similar)
  • Bachelor's degree in Nursing, Social Work, or related field (preferred) A teritary qualification is efficient for the role.
  • Minimum of 3 years' experience in Aged Care coordination or related role
  • High level written and verbal communication skills
  • Excellent organisational skills and ability to prioritise competing tasks
  • Ability to work within a team and also autonomously
  • Strong networking skills and experience
  • Excellent decision making, analytical and organisational skills
  • Familiarity with budget management within allocated packages
  • Experience in conducting Aged Care assessments and developing care plans
  • Knowledge of Aged Care Services, Aged Care standards, community resources and service networks for older Australians and their carers
  • Ability to adhere to quality standards and compliance requirements
  • Computer literacy, including Excel, databases and reporting
  • Understanding of Aged Care Sector, Commonwealth Home Support Program Packages, Aged Care Standards, Code of Conduct and legislative frameworks
