My Aged Care Provider

FOCUS Connect, an approved My Aged Care Provider, is dedicated to supporting older Australians through the Support at Home program (formerly Home Care Packages) and the Commonwealth Home Support Programme (CHSP).  We empower individuals to remain safe, independent, and connected in their homes and community.

Support at Home (SAH) and Commonwealth Home Support Programme (CHSP)

FOCUS Connect delivers government-funded Support at Home (SAH) and Commonwealth Home Support Programme (CHSP) services across South West Sydney, including Wollondilly, Wingecarribee, Campbelltown, Bankstown, Camden, Fairfield, and Liverpool, as well as Northern Sydney suburbs such as Ryde, Hornsby, Pymble, and surrounding areas.


We provide tailored in-home support that helps older Australians live independently, safely, and confidently in their own homes and communities.

Support at Home (formerly HCP)

As an approved My Aged Care provider for all care levels, FOCUS Connect delivers Support at Home services across Macarthur, South West Sydney, and Northern Sydney Suburbs.


Our comprehensive support includes:

  • Clinical care and nursing support provided by FOCUS Connect’s qualified nurses
  • Personal care (showering, dressing, grooming)
  • Domestic assistance and meal preparation
  • Transport to appointments, shopping, and community activities
  • Social support, companionship, and multicultural support groups
  • Respite care for carers
  • Coordinated supports such as:
  • Allied health and restorative therapies are arranged through trusted external providers
  • Assistive technology and home modifications
  • Gardening, lawn care, and home maintenance

Commonwealth Home Support Programme (CHSP)

FOCUS Connect also delivers a wide range of services under the CHSP program to help you live independently at home while staying socially connected.


Our CHSP services include:

  • Domestic assistance (cleaning, laundry, household tasks)
  • Personal care (showering, dressing, grooming)
  • Transport to appointments, shopping, and community activities
  • In-home respite for carers
  • One-on-one social support and companionship
  • Group social support programs
  • Multicultural social support groups (with weekly activities in Spanish, Egyptian, Laotian, Chinese, Arabic, Vietnamese, Mauritian, Filipino, and more)
  • Group outings and excursions for eligible participants


We deliver CHSP services across South West Sydney and Northern Sydney, supporting older Australians to stay safe, active, and connected.
