FOCUS Connect is excited to announce its participation in the trial rollout of the Assistive Technology and Home Modifications (AT-HM) Scheme, a key feature of the upcoming Support at Home program. Starting 1 July 2025, the Support at Home program will replace the Home Care Packages and Short-Term Restorative Care Programme, with delivery planned in two stages from July 2025. This new program aims to enhance access to services, equipment, and home modifications, helping seniors remain healthy, active, and socially connected.
